The New Dimension Adult Foster Care Program is just what is needed to help many people stay in their community settings. Aftercare will help coordinate your healthcare services as well as make sure your social needs are met in a warm and caring home. If you are currently enrolled in the Massachusetts Medicaid Program, Aftercare can help match clients with appropriate caregivers. If you are a potential client or caregiver please contact our office and we will be honored to help.

If you live with a person who is attending to these needs and is not an immediate family member (mother or father, biological or adoptive) or (legally married couple) you may be eligible for this program.
